Coronary stents

Drug-eluting stents that hold a treated coronary artery open after angioplasty.

How a stent is placed

  1. 01A thin guidewire is passed along the artery and through the narrowing.
  2. 02A balloon catheter carrying the crimped stent slides along the wire to the narrowing.
  3. 03The balloon is inflated. It expands the stent and presses the plaque against the artery wall.
  4. 04The balloon and wire are taken out. The stent stays, holding the artery open so blood flows freely.

NC balloons

Non-compliant balloon catheters that keep their size at high pressure. They are used to expand a stent fully after it is placed, and to open hard, calcified narrowings.

How an NC balloon is used

  1. 01At a hard, calcified narrowing, a newly placed stent can stay too narrow in the middle.
  2. 02The NC balloon slides along the guidewire into the stent.
  3. 03It is inflated at high pressure. A non-compliant balloon keeps its size, so it opens the narrow part without over-stretching the artery.
  4. 04The balloon and wire are taken out, leaving the stent evenly and fully expanded.

PTCA guidewires

Coronary guidewires that cross the lesion and carry balloons and stents to it. The BMW is the everyday workhorse wire; Whisper, All Star and Cross-IT wires are used where a lesion needs a different tip, support or coating.

How a guidewire is used

  1. 01A guiding catheter is placed at the opening of the left coronary artery.
  2. 02The guidewire is passed through the catheter and steered into the LAD.
  3. 03Its tip is advanced carefully across the narrowing.
  4. 04The wire now works as a rail: balloons and stents slide along it to the narrowing.

OCT systems and catheters

Optical coherence tomography (OCT) shows the inside of a coronary artery in fine detail. An imaging catheter scans the artery wall with light, and the OCT system turns the scan into cross-section images.

How OCT imaging works

  1. 01The imaging catheter slides along the guidewire, past the section to be examined.
  2. 02A short flush of contrast clears the blood from the artery for a few seconds.
  3. 03The lens spins and is pulled back automatically, scanning the artery wall with near-infrared light.
  4. 04The OCT system shows the section as cross-section images: the narrowing, the plaque and, after stenting, how well the stent has expanded.

How a pacemaker is implanted

  1. 01Through a small cut below the left collarbone, the lead is passed into a vein.
  2. 02The lead is guided down the vein into the heart, and its tip is fixed in the right ventricle.
  3. 03The lead is connected to the pacemaker, which is placed in a pocket under the skin.
  4. 04When the heart beats too slowly, the pacemaker sends a small electrical pulse and the heart contracts.
